Hello doctor,

I am in desperate need of medical dermatological advice for my skin condition. My skin is extremely oily right now, but it is also dry. I have tried charcoal masks and Niacinamide sheet masks, but nothing has been able to remove the extreme oiliness that makes me break out and feel uncomfortable on my face. When I do not use a moisturizer or only a little, my skin feels tight and dry, but when I look in the mirror, there are still oils flowing out excessively, even when I feel really dry in the face and wipe away the excess oil. They appear like dry flakes on my skin that peel off slowly and the appearance of what looks like dead skin. So I am quite concerned about what I can do about my skin. I use both Kojic acid bar soap and Kojic acid body lotion.

I have been trying to get rid of my facial hyperpigmentation, but as a person with brown skin, it seems to have gotten worse to the point where my t-zone is lighter and more of my tone, but other areas are very dark, including the areas around my mouth and chin, as well as near my hairline. I do not feel I have any chronic illnesses, except sinuses, which I doubt are related to my issue. I have always had oily and dry skin at the same time, but it was in moderation before and did not feel as uncomfortable on my skin as it does now as many oils appear on my skin. Kindly help.

From your description, your condition appears to be seborrheic dermatitis. I suggest you consult your specialist doctor, discuss with him or her, and take the medicines with their consent. In addition, you can do the following:

  1. Apply Metronidazole (Nitroimidazole antimicrobials) 0.075 percent gel at night for ten days.

  2. Apply Caprylic or Capric triglyceride lotion immediately after bathing for 20 days.

  3. Apply Lactic acid soft cream or Cetyl alcohol or aloe vera cream twice daily for two weeks.

  4. Use sodium palmate soap for bathing for two months.

  5. Wash your face with Cocamidopropyl Betainamide MEA Chloride face wash twice daily for two weeks.

  6. Use skin moisturizer frequently.

  7. Avoid scratching and rubbing of lesions.

  8. Avoid known triggers.

  9. Avoid oily or greasy cosmetic products.

  10. Avoid exposure to extreme weather conditions.
