What causes drowsiness and leg swelling?

Hello doctor,

My father is 42 years of age, and he has a congestive and congenital heart disease by birth. He visited many doctors, but all of them suggested that there is no need for any immediate treatment and that he can live with it. He is now under medications, and he is doing well. The reason I am addressing you is that my father recently found his urea and creatinine to increase a bit, so we consulted a nephrologist. The doctor said that this is because of his heart problem, the blood does not reach his kidneys properly. They think it is because of dehydration and fluid retention, which has caused an increase in the urea level. But from a month, he is feeling extremely drowsy, which he often experiences, as his body retains fluid, but it decreases again through taking medicines such as Lasix. The only issue now, is that his legs and stomach are swollen due to water retention, and he is feeling very drowsy, and he cannot concentrate on his work. How to effectively treat his drowsiness and swelling? Is there any alternate treatment for his condition?

How to deal with sinus tachycardia with normal blood pressure and blood tests?

Hi doctor,

I have tachycardia with average heart rate between 110 to 130 beats per minute while standing. On the other hand, while lying down it is just 80 to 100 beats per minute. I am easily fatigued and have mild chest pain. ECG shows sinus tachycardia, abnormal depolarization nonspecific, poor R progression in right pericardial leads, QRS within normal limits, and prolonged QT interval. My blood pressures are normal. I have been experiencing it for the past seven days. My blood tests are all normal.

Can beta blockers help treat PVC after starting Methadone?

Hi doctor,

I am a 35 year old male, and I am currently on Methadone for chronic pain. I currently have PVC, and my last EKG showed a long QT (over 500 ms). The doctor recommended that I take Magnesium and Potassium supplements as he believes that Methadone may be reducing my magnesium and potassium levels. It has been a week now since I have been following the regime and currently the PVC is nearly gone. I do not have the option of reducing the Methadone due to my injury. The doctor said he wanted me to get an opinion from a cardiologist if I may need a beta blocker or not. He wants to wait to see how the supplementation works out first. Is there any advice you can give me? I get an EKG done twice a year.

What causes frequent chest pains and shortness of breath?

I am an 18 year old male, and I am overweight. I have been having sharp upper right chest pains, that are very frequent. I did not have them before, but I started getting them recently. At first, I was having shortness of breath, and it felt as if someone was standing on my chest. But now, I have bad sharp pains at night. They come and go intermittently.
