For an unstable cervical spine, posterior cervical instrumentation is done. It uses wiring, rod, plates, and screw fixation. In the case of multi-level instability, a pedicle screw fixation provides a more biomechanical advantage. Posterior cervical wiring was done previously for fixation. But nowadays, posterior cervical plates and screws are placed. Posterior cervical plates are riskier as placing screws can damage the vertebral artery and nerve root, but it provides better stability.
